^*1 This is not medical insurance, but pre-payment for Doctor’s visits at considerable savings to you.

² Only Doctor’s visits are covered in the Pre-Paid Plan. One single payment covers as many visits as you need in one calendar year to any of our Family Doctors in our 5 offices in Queens and Brooklyn. Any other services needed, including: PPD, Labs, Vaccines, EKG, PFT, Audio etc. will be at extra charge.  You must pay for the entire year at time of registration by cash, check or credit card. 30 days advance notice required for cancellation. Unused portion of payment will be refunded pro-rata.
